Collection and Use of Protected Health Information To provide services to you, we are required to collect information about you and your health care needs from you and others, including insurance companies, hospitals, physicians, family members, and other caregivers.
As your home health care provider, CarePoint will not sell your PHI. To ensure optimal care and treatment, we must share information about you and your health care needs for treatment, payment, and health care operations.
