Privacy Notice for CarePoint Home Health Client

This notice describes how medical information about you may be used and disclosed, and how you can access this information. Please review it carefully.

CarePoint Home Health Agency (CHHA) is committed to respecting your confidentiality and privacy. We have established policies and procedures to protect your protected health information (PHI) and to restrict how it is used. PHI includes any medical information regarding your care and treatment that may identify you, such as your name, address, phone number, social security number, email address, diagnosis, and other similar information you provide during treatment. PHI may be in both paper and electronic formats.

Federal and state governments oversee home health care to ensure that we provide quality services while keeping your PHI secure and confidential.

Collection and Use of Protected Health Information

To provide services to you, we are required to collect information about you and your health care needs from you and others, including insurance companies, hospitals, physicians, family members, and other caregivers.

As your home health care provider, CarePoint will not sell your PHI. To ensure optimal care and treatment, we must share information about you and your health care needs for treatment, payment, and health care operations.

 

Examples of How We Use Your Protected Health Information Include:

  • Communication: Sharing information with your physician or other members of your healthcare team to keep your care plan up to date.
  • Payment: Sharing information with insurers or health plans to facilitate payment for services rendered.
  • Training and Quality Improvement: Using information for internal reviews, teaching, and training purposes to ensure quality care.
  • Recommendations: Informing you about potential treatment options, products, or services offered by CarePoint or third-party providers.
  • Legal Requirements: Disclosures to a coroner, medical examiner, or organizations involved in organ donation when applicable.
  • Business Associates: Sharing information with non-employees who assist us in your care, all of whom are required to follow the same privacy standards as CarePoint.

 

Your Privacy Rights

You have the right to:

  • Access Your Records: Request to see or obtain copies of your medical file. We will provide access within 30 days of your request.
  • Request Communication Preferences: Ask us to communicate with you in specific ways or at specific locations.
  • Restrict Access: Request that certain individuals not have access to your PHI.
  • Notification of Breach: Be informed if there has been unauthorized access to your PHI.
  • Amend Your Records: Request corrections to your information if you believe it is inaccurate or incomplete.
  • Accounting of Disclosures: Request a list of disclosures of your PHI over the past six years.
  • Opt-Out of Fundraising: Contact us to refuse participation in fundraising activities conducted by CarePoint.

 

Changes to This Notice

We may update this Privacy Notice periodically. Copies of the updated notice will be available in our service office and posted on our website. If you would like a paper copy, please ask your Clinical Manager.
