This notice describes how medical information about you may be used and disclosed, and how you can access this information. Please review it carefully.
General
CarePoint Home Health Agency (CHHA) is committed to respecting your confidentiality and privacy. We have established policies and procedures to protect your protected health information (PHI) and to restrict how it is used. PHI includes any medical information regarding your care and treatment that may identify you, such as your name, address, phone number, social security number, email address, diagnosis, and other similar information you provide during treatment. PHI may be in both paper and electronic formats.
Federal and state governments oversee home health care to ensure that we provide quality services while keeping your PHI secure and confidential.
To provide services to you, we are required to collect information about you and your health care needs from you and others, including insurance companies, hospitals, physicians, family members, and other caregivers.
As your home health care provider, CarePoint will not sell your PHI. To ensure optimal care and treatment, we must share information about you and your health care needs for treatment, payment, and health care operations.
Examples of how we use your protected health information
- Communication: Sharing information with your physician or other members of your healthcare team to keep your care plan up to date.
- Payment: Sharing information with insurers or health plans to facilitate payment for services rendered.

- Information related to current or former employees of CarePoint (unless submitted through our job application platform)
Personal information you provide
We may request certain personal information when you submit an online request for information about CarePoint services; sign up for email or text message communications from us; register for an online account; make online payments for Services; inquire about career opportunities or submit an employment application; make a charitable contribution to CarePoint; or request information about our partner programs.
Depending on your request, we may ask for the following information: name, email address, telephone number, postal address, care recipient’s name and zip code, type of service of interest for the care recipient, payment card information for online payments (processed by a third-party provider), and employment application details (if applicable), including demographic information, education, and work history.
Information collected automatically on our Sites
We and our authorized third-party providers automatically collect data when you visit our Sites using cookies, pixels, web beacons, and similar tracking technologies (collectively, “cookies”). If you provide personal information related to our Services, we will use your information to respond to your inquiries and fulfill your requests; send you updates about our Services; process payments for Services (using a third-party processor); send email marketing communications, including newsletters and promotions unless you opt out; and comply with legal obligations and respond to government requests.
If you apply for a caregiver position using our online application platform, we will use the information you provide to evaluate your application, communicate during the application process, assist with onboarding, and consider your application for alternative positions.
